いつも おせわに なっています("[I/Ingroup member(s)] is/are being helped/cared for.)

This is a polite expression used to greet someone from outside of your family members. Literally, it means the speaker or the members of one's own family (or ingroup) is/are being helped by the person you are greeting. This is a customary way of showing one's appreciation toward the person, and it is always said regardless of whther that person is actually being helpful or not.
